WakeMed Key Community Care (a collaboration between WakeMed Health & Hospitals, WakeMed Physician Practices, and Key Physicians) brings together a significant network of more than 220 independent primary care physicians with a leading independent health system with another 250 primary care and specialty providers. The WKCC will facilitate patient access to a coordinated health care team focused on delivering efficient, quality care at a reasonable cost. Streamlining communication with the patient and between health care providers will reduce duplication of services, limit unnecessary tests, ease transitions from hospital to home and help patients maintain good health. "This Accountable Care Organization will uniquely benefit our community by bringing together valuable health resources in a more integrated way that will ultimately lead to better health. This partnership offers new opportunities for providers to work closer together to help patients manage and improve their health," commented Donald Gintzig, WakeMed president & CEO. "Patients will benefit from population health strategies aimed at providing a more seamless coordination of inpatient and outpatient services, clinical resources, and physicians. This is a profound opportunity to help us achieve a fundamental aspect of our mission...improving the health of our community". Wake Orthopaedics is extremely pleased to partner with the Key Physicians network to better the health of our community.
